DaVita at Home modalities

DaVita at Home enables you to offer your patients a broader menu of treatment choices and may provide greater operating flexibility for your practice than facility-based dialysis. Medicare has considered the importance of these modalities in their rule-making regarding MCP reimbursement.

Choice of modalities

Having a choice of modalities allows a physician to pick a treatment that best fits the clinical and social needs of the patient.

Peritoneal dialysis

  • Traditional in-home, patient independent, automated cycler (CCPD) and CAPD
  • 24/7 nurse support just a phone call away
  • Monthly multidisciplinary patient care meetings

Home hemodialysis

  • In-home, patient semi-independent with 24/7 nurse and biomed support just a phone call away
  • On-site dialysis partner required in most cases
  • Monthly multidisciplinary patient care meetings
  • Types of home hemodialysis:
    • Traditional (3x/week)
    • Short daily (5-6x/week)
    • Nocturnal (3 or more nights a week)

Self Care

In-center hemodialysis, patient supervised by a nurse, flexible timing, 3x or more a week. This is an option for patients as a transition to home-based dialysis or for patients who are not comfortable dialyzing alone, whose dialysis partner is temporarily unavailable or whose medical condition may make them inappropriate for unsupervised dialysis. (Not available in all areas.)
