Personalized patient education

We believe in the value of educating you, the patient, to understand and even administer your own therapy. In addition to providing you with high quality services, your HomeChoice team is responsible for enabling you and/or your caregivers to assist in or administer your infusion therapy.

Over time, our goal is to make you as independent of an outside resource as possible. How do we accomplish this?

Step 1: Assessment of Learning Needs

Based on your unique plan of care, your team will determine what you need to learn in the intermediate and long term.

Step 2: Individualized Training Sessions

Your HomeChoice professional will clearly demonstrate infusion therapy techniques using concise, organized verbal and written instructions.

Step 3: Verify Competency

Once training is complete, you or your caregiver must demonstrate competency in your infusion therapy under the supervision of the registered nurse.

Step 4: Follow-up Visits

Once you or your caregiver proves that you have learned the correct administration techniques for your particular therapy, the HomeChoice nursing staff will continue to perform follow-up visits as needed. HomeChoice has a Registered Nurse and a Registered Pharmacist on call 24 hours a day, 7 days a week for any questions or needs that may arise.
