Let us know seven days before you run out, and we will send these medications.

You may call DaVita Rx℠ toll-free at 1-888-DAVITA-RX (1-888-328-4827) to order refills or to speak to your pharmacist. In addition, you can track your DaVita Rx prescriptions when you sign up for the DaVita Health Portal™, a benefit exclusively for DaVita patients.
All of your prescriptions written by any of your prescribing physicians should be faxed from their offices. Please carry your DaVita Rx card with your DaVita Member Patient Identification (MPID) number to all of your doctor’s appointments.
Prescriptions (such as antibiotics that you need to have filled the same day to treat an acute condition such as an infection) should be filled at a local pharmacy.
DaVita Rx will not dispense Schedule II narcotics such as oxycodone, opiates or methadone.
Many insurance companies have different co-pay levels for name brand drugs. In order to help you manage your prescription medication costs, DaVita Rx’s policy is to dispense generic medications for name brand medications whenever possible. In addition, some insurance plans, such as many Medicaid programs, may only cover generic medications unless a physician provides sufficient justification.
