Tell us in 250 words or less your uplifting, heartwarming and inspiring true story about someone in the chronic kidney disease or dialysis community who makes a difference to those around them. Nominate a kidney disease or dialysis patient, a caregiver, friend, spouse, health care professional* or anyone — it could even be you!

You help decide who's the Kidney Idol Four finalists will be chosen from all of the entries and posted at DaVita.com. The winner of the cruise will be the finalist who receives the most votes from DaVita.com visitors. You help choose the winner by voting for your favorite!
