Credentialed Physicians
DaVita’s credentialing is now available online:
- Fill out the appropriate form by clicking one of the links below
- Email the form to the appropriate contact to get the credentialing process started
To start the process, fill out the new professional request form.
Already on staff and need to add a facility?
Click here to open the form, click on the tab labeled "multiple" and fill in the requested information.
Initial credentialing is required for all DaVita® physicians and Allied Health Professionals (AHPs) new to DaVita. Re-credentialing or reappointment is required every two years. If a practitioner requests privileges at an additional facility in between appointments, they will need to go through an abbreviated credentialing process as a facility add request.
