Q: What is DaVita Rx?
A: DaVita RxSM is a full-service pharmacy designed specifically for dialysis patients. All prescriptions are dispensed by licensed DaVita Rx pharmacists and checked carefully for duplicates and for drugs that are unsafe when taken together.
Q: Who can use DaVita Rx?
A: DaVita Rx provides pharmacy services to hemodialysis, home hemodialysis (HHD) and peritoneal dialysis (PD) patients.
Q: What insurance does DaVita Rx accept?
A: DaVita Rx accepts most commercial and government plans.
Q: How much does it cost to use DaVita Rx?
A: There is no delivery fee; however, in most cases, prices (including co-pays) are set by your insurance company, not by your pharmacy.
Q: Is a prescription needed?
A: Yes, just like with any other pharmacy. All prescriptions written by your non- DaVita doctors (like your cardiologist or general practitioner) can either be:
Where state law permits, prescriptions from your DaVita nephrologists can be faxed or phoned into the pharmacy directly from your clinic.
Q: I’d like to use DaVita Rx for all of my existing prescriptions. Do I have to have my doctor write new prescriptions?
A: Not in most cases. We work directly with your physicians for existing prescriptions. However, your doctor will have to fax or call in all new prescriptions, or prescriptions with no refills.
Q: I have prescriptions from many different physicians. Can I get all my prescriptions through DaVita Rx?
A: Yes. DaVita Rx can fill prescriptions from any physician. DaVita Rx carries all prescription medications, with the exception of some controlled substances such as morphine and oxycodone. DaVita Rx also offers many over-the-counter (OTC) medications such as aspirin and vitamins. Q: What do I do with my prescriptions written by the emergency room doctor or during my hospital stay?
A: For one-time refills (for example: antibiotics, pain medications, etc.) take the prescriptions to your local pharmacy or if the prescription has refills, please take it to a retail pharmacy for a first fill then call DaVita Rx and we will transfer the remaining fills. If the emergency room or hospital physician is able to submit electronic prescriptions, please ask the physician to send them directly to DaVita Rx.
Q: What if DaVita Rx does not have all of my medications in stock?
A: Because we specialize in serving dialysis patients, DaVita Rx will typically have any needed medications in stock.
Q: What if I need to receive a medication right away?
A: Any medication that you need on the same day, such as an antibiotic or a new heart medication, should be filled through a local retail pharmacy to ensure that you receive it as soon as possible. If you will need this medication on a regular basis, ask your physician to write two prescriptions for you: one to fill immediately at a local pharmacy and one for DaVita Rx to fill for you in the future.
Q: What guidelines does DaVita Rx follow in filling prescriptions?
A: A DaVita Rx pharmacist will review all of the prescriptions that you provide for accuracy, to check for possible duplicate drugs/therapeutic equivalents, and to ensure that there will be no adverse reactions.
Q: What is DaVita Rx’s generic policy?
A: To help you save money, DaVita Rx will substitute generics whenever possible, unless otherwise requested by you or your physician. Generics are defined by the Food and Drug Administration (FDA) as drugs that are identical or bioequivalent to a brand name drug in dosage, form, safety, strength, performance and intended use.
Q: How do I pay for medications?
A: DaVita Rx offers three (3) flexible payment options:
1) Authorized payment by major debit/credit cards (Visa, MasterCard, American Express and Discover); your card will be billed when your medications are dispensed.
2) Pay by check or money order upon receipt of prescriptions (an invoice and self-addressed envelope is provided).
3) Pay at the end of the month when you receive your itemized statement at your billing address.

Q: What are the hours of operation for DaVita Rx?
A: DaVita Rx business hours are Monday – Friday. 8 a.m. – 10 p.m. Eastern Time (ET), and Saturday, 8 a.m. – 5 p.m. Eastern Time (ET). There is a pharmacist on call 24 hours a day, seven days a week if you have an urgent question about one of your medications.
