Tammy Elzy Director of Physician Services	Tammy Elzy Tammy is the Director of Physician Services where she oversees the DaVita SOURCE™ team which provides physician and mid-level recruitment services to DaVita affiliated practices nationwide. For more than a decade, she has been working with nephrology practices affiliated with DaVita and has a proven track record for success in all areas of DaVita she has been involved with. Her experience and knowledge has assisted the DaVita SOURCE team in placing more than 150 physicians over the past three years and on track for another 60 in 2009. Tammy covers the Midwest region extending from Minnesota to Texas and the state of Florida. Prior to joining DaVita SOURCE, Tammy was part of the DaVita Clinical Research® (DCR) team from 1998 to 2006. In her first three years she provided support to the Phase I clinical research operations. In 2001 she helped start the Site Management Organization, a national division of DCR, where she successfully launched Phase III and IV clinical trials with industry leading biotechnology companies. She was responsible for the oversight of all clinical research trials conducted nationally within DaVita in more than 100 locations. She leveraged internal and external relationships along with strategic planning to promote growth of the organization. In her tenure with DCR she increased revenue consecutively for four years with increases as high as fifty percent. She is married with one son. Her hobbies include: gardening, running, biking, sports photography and reading. Tammy's office is located in Lindstrom, Minnesota, where she also resides.

Louise Jacobson Physician Resource Territory Manager	Louise Jacobson Louise is the Physician Resource Territory Manager for the Southeast region where she is responsible for assisting DaVita affiliated groups and fellowship programs with physician and mid-level recruitment. Louise has over 20 years of extensive health care recruiting experience which has given her a unique and thorough understanding of today's complex medical market allowing her to easily build mutually beneficial relationships with both candidates and clients for DaVita SOURCE. Prior to joining DaVita SOURCE, Louise owned and managed a health care recruitment firm for more than 10 years in North Carolina. As a Principle recruiter, her firm initially focused on Mid-level providers nationwide and later expanded to include hospital executives and physicians as her client base. Her clients ranged from the solo practices and small community hospitals to large groups and tertiary care centers. Before owning a healthcare recruitment firm, Louise grew in the field of recruiting as a consultant for United Medical Associates, NC. In this position, she gained experience in preparing state and national bids, the requirements of the state licensing boards and processing J1 visas. She currently resides in Kernersville, North Carolina, where her office is based. Louise has two children and enjoys volunteering locally and reading.

Cedric Joins Physician Resource Territory Manager	Cedric Joins Cedric is the Physician Resource Territory Manager for the West Coast region where he is responsible for assisting DaVita affiliated groups and fellowship programs with physician and mid-level recruitment. He has worked in the medical community in a variety of capacities for more than 25 years and for the last 3 years he has dedicated his time exclusively to nephrology and helping DaVita medical directors prosper by finding and hiring the best and brightest "right fit" nephrologists for their practices. Prior to joining DaVita, Cedric worked in the financial services industry as a Financial Advisor and Retirement Planning Specialist at Morgan Stanley®, Beverly Hills and as an Advisor and Business Banking Officer for Citigroup® where he advised physicians on everything from cash management and retirement plans to securing business loans. Before then, he was a Medical Sales Consultant with Haemonetics Corporation® where he was responsible for onsite in-service and promoting a complete line of blood bank products for intraoperative autotransfusion and other therapeutic applications. Cedric also worked as a Medical Systems Specialist for Alcon Laboratories® where he sold the Ivy Practice Management System® throughout much of California and trained medical professionals from various specialties on its software applications. Cedric first established his career in territory management while employed by Bristol-Myers Pharmaceutical Group where he managed territories for Boston and Los Angeles. He later worked as a representative for their microsurgical division, Xomed-Treace®, where he routinely demonstrated the use of surgical products and instruments both in and out of the operating room. Cedric lives in Los Angeles, California with his wife and two children. When not working, Cedric enjoys singing.

Amy Sabin Barrow Physician Resource Territory Manager	Amy Sabin Barrow Amy is the Physician Resource Territory Manager for the Northeast region. Based in New York City, she provides recruitment services to DaVita affiliated groups and fellowship programs for both physicians and mid-levels in nephrology. Amy comes to DaVita SOURCE with over ten years of healthcare business experience in a wide range of services including telemedicine, nonprofit hospital development and marketing, financial administration, practice management, privatized Medicare managed care and physician recruitment. Her office is located in Brooklyn, New York where she’s made her home for the last five years. Amy is active in the theater with her husband Scott.

Melissa Schares Recruitment Coordinator	Melissa Schares Melissa is the Recruitment Coordinator providing research and system support for the DaVita SOURCE team. She has been with DaVita for over 5 years. Prior to DaVita SOURCE, Melissa worked for DaVita Clinical Research (DCR) and the Site Management Organization (SMO) division as the Regulatory Affairs Coordinator. She helped increase revenue for two consecutive years and she assisted in the site placement and management for all clinical trials conducted in over 100 DaVita locations. She also provided and supervised trial documents, certifications for research staff, Sponsor and IRB, to ensure that all trials met Federal and International requirements for medical research. She was also the administrator for DCR's internal corporate grant program. Melissa's office is located in Lindstrom, Minnesota. She is a proud mother of one and an avid lover of music and photography.

Michelle A. Curtis Business Analyst	Michelle A. Curtis Michelle is the Manager for the Fellowship Division. She joined the DaVita Practice Strategy Group in 2006 as a business analyst for DPSG’s summit groups where she focused on market research and branding efforts. As of early 2008, her efforts have been with the fellowship division where she provides support for DaVita SOURCE marketing campaigns, the annual Leaders in Nephrology conference, the annual ASN conference and works directly with nephrology fellowship programs across the country on educational and career development for nephrology fellows. Prior to DaVita, she was an Account Manager for a brokerage firm that specialized in commercial insurance for mid-sized corporations. Previously, she served as part of an advertising sales team for a variety of nationally-based pharmaceutical companies. She currently resides in El Segundo, California. When not working, Michelle enjoys time at the beach, practicing yoga and eating sushi.

Robin-Gail Paraiso Business Analyst	Robin-Gail Paraiso Robin is the Business Analyst for the Fellowship Division. The Fellowship Division provides physician recruitment services and resources to direct a fellow’s career development. She is responsible for the candidate selection, initial screening and referral process for graduating fellows. Before joining DaVita, Robin worked for Kaiser Permanente Medical Group as a Deployment Analyst supporting their Electronic Medical Record, KP Health Connect. She traveled throughout Southern California, implementing both Inpatient and Ambulatory Go-Lives. Serving as a floor coordinator for the deployment team, she focused on helping physicians, nurses and staff transition from paper to electronic charting. She currently resides in Long Beach, California. When not working, Robin enjoys a funny movie, a great work out and eating dark chocolate.
