Don’t be concerned if you still have a few days’ supply of pills; we want to make sure you don’t run out of your medication.

2) When it comes to your “Take as Needed/Call for Refill” medications, give us a call!

Let us know seven days before you run out, and we will send these medications.

3) The Yellow Page is very important.

Make sure you review this every week and notify the pharmacy about medications you do not need. Call toll-free at 1-888-DAVITA-RX (1-888-328-4827).

4) Receive your medications on schedule and choose your payment method. There are three (3) different ways that you can pay for DaVita RxSM medications:

  1. You can pay when you get your medications.
  2. You can pay at the end of the month.
  3. You can set up automatic payments.

Important information about DaVita Rx


All of your prescriptions written by any of your prescribing physicians should be faxed from their offices. Please carry your DaVita Rx card with your DaVita Member Patient Identification (MPID) number to all of your doctor’s appointments.

Prescriptions (such as antibiotics that you need to have filled the same day to treat an acute condition such as an infection) should be filled at a local pharmacy.

DaVita Rx will not dispense Schedule II narcotics such as oxycodone, opiates or methadone.

Many insurance companies have different co-pay levels for name brand drugs. In order to help you manage your prescription medication costs, DaVita Rx’s policy is to dispense generic medications for name brand medications whenever possible. In addition, some insurance plans, such as many Medicaid programs, may only cover generic medications unless a physician provides sufficient justification.

Refill Management System medications

DaVita Rx offers a unique opportunity for you to have your medications refilled automatically with minimal effort from you:

  • Medications on the Refill Management System will be sent to you a few days before you run out of medication.
  • You may still have a few days’ supply left when you receive the next month’s medication, but this is to ensure that you never run out.
  • Please be sure to review your Yellow Page every week and call us if you will not need your Refill Management System medications.

Stopping Refill Management System medications

Be sure to call the pharmacy if you are filling at a local pharmacy, on vacation, or your doctor has changed your prescription dosage. If you have a medication and you need to stop the Refill Management System, do one of the following:

  • Call us to let us know you no longer prefer using the Refill Management System and request that the Refill Management System is turned off.
  • Fax in the Yellow Page and indicate that the Refill Management System is no longer preferred for a specific medication.
  • Call us and have the medication’s delivery date changed to ship before you run out of medication.

“Take as Needed/Call for Refill” medications

You may elect to have some or all medications sent to you only when you request them.

  • These medications will be listed separately on your Yellow Page.
  • Please let us know at least seven (7) days before you run out of these medications, so we will have enough time to get them to you.

Yellow Page

The Yellow Page is a very important communication from the pharmacy regarding your medications. It is included with every prescription order.

Things to remember:

  • Medications listed under “Take as Needed/Call for Refill” medications on the Yellow Page will not be sent automatically. You must call DaVita Rx (1-888-328-4827) seven (7) days before you run out to ensure you receive in time.
  • If your medication is on the Refill Management System and you no longer want the medication, please call the pharmacy and let us know.
